چکیده

The purpose of this study was to evaluate the design of a cup using finite element method and to analyze possible effects of joint loading postoperatively, and its initial mechanical stability, so as to direct its further optimization. Finite‐element (FE) models of the cup with three wings and the hemispherical cup were created to calculate the stress patterns during a normal gait cycle. The stress in the acetabular components were analyzed and compared. The FE analysis demonstrated that all kinds of acetabular components had the same trend for stress and strain. The stress of the wings increased gradually from rim to root. Its peak stress was significantly lower than the yield force of the Co‐Cr‐Mo alloy at the joint between the wing and the shell. The graft portion near the acetabular component was subjected to higher stress conditions. The contact stresses were found to be decreased with a reduced abduction angle of wings. The cup with wings of abduction angle of 15° had lower stresses compared with other cups. The cup with wings is a reliable option for the reconstruction of the acetabulum with extensive bone socket defects. The reduced abduction angle of wings helps to decrease the stress of the cup with wings. The FE analysis is a useful tool with which to address these issues.

The majority of acetabular defects can be reconstructed with a hemispherical acetabular component alone. However, severe bone loss encountered at the time of revision acetabular surgery can result in compromised initial fixation leading to component micromotion, failure of bone ingrowth, and mechanical failure. Porous acetabular augments offer a potential solution to enhance initial component f...

Hip Arthroplasty and its Revision in a Child: Case Report and Literature Review

Juvenile idiopathic arthritis is the leading cause of hip replacement in young children. However, arthroplasty in this population is challenging with several concerns about quality of the growing bone, young age for revision surgery, and difficulties in potential several revisions. In this study we introduce a case of a 12-year old who is one of the youngest patients to undergo revision hip art...

This work presents a numerical approach in order to predict the influence of implant material stiffness in a femoral component design when submitted in compression. The implant success depends on the transferred load to the neighboring bone. The finite element method can be used to analysis the stress and strain distribution in the femoral component allowing to improve the implant selection. Fo...

Managing bone loss in acetabular revision.

The management of bone loss encountered during acetabular revision remains challenging. In order to obtain a successful surgical result, preoperative planning is required to estimate the severity and location of bone defects. Most acetabular revisions can be treated with the use of a cementless hemispherical component. However, a successful surgical reconstruction requires component stability. ...
